Northern Light Health is seeking a community health worker to help patients overcome barriers, access resources, and take meaningful steps toward better health and well-being. In this role, you’ll build trusted relationships with patients of Northern Light Maine Coast Hospital in Ellsworth in their homes, healthcare settings, and communities while helping them: Connect with healthcare and community resources Set and work toward health goals Access benefits, food assistance, housing resources, prescriptions, and other support Navigate appointments and the healthcare system Address social challenges that may impact their health We’re looking for an empathetic listener, strong communicator, and natural relationship-builder who is comfortable with outreach and home visits.
